Well, after two somewhat questionable choices, we get an actual scary enemy.  The Dead Hand is a mini-boss in Ocarina of Time, located in the Kakariko Well and the Shadow Temple.

This twisted creature scared many a young gamer in the day.  When you see it at first, all you’ll see is the hands rising up.  But soon enough, the body appears and lunges at you, mouth wide open.

 

6. Giygas from EarthBound

There.  That’s… creepy.

Giygas is the antagonist from the first two Mother games.  You let him go after defeating him in the first game.  In EarthBound, Giygas returns stronger and would eventually conquer the world in ten years.  Near the end, Dr. Andonuts will say how he believes that the attacks originated in the past.  After transferring the spirits of Ness, Paula, Jeff, and Poo into robots, the kids travel back to the past to find Giygas.

…in what appears to be a giant uterus with Ness’s face up front.

Giygas is unsettling to look at and the creepiest final boss in the opinion of a number of gamers.  Based on what Shigesato Itoi was trying to convey in this fight, I think anyone that has played it needs psychiatric help.

 

5. The Gatherers from Amnesia: The Dark Descent

Servants of Alexander of Brennenburg, these creatures used to be soldiers that abandoned their post.  Lost in the dark forest and damned to roam the grounds, the soldiers became disfigured and empty of essence, possibly from Alexander’s poisoned wine.

The Gatherers are some of the scariest creatures in modern games.  If you look at them, you begin to lose your sanity.  If you try to run from them, they will give chase; the farther away, the quicker they’ll run.  You won’t be able to attack them, but they will kill you.
